For client convenience I have included my intake and disclosure paperwork to print, fill out and bring with you or email to me by your first appointment.
Documents to bring with you on/or email me before your first Office or Tele-Health appointment:
1. New client information packet (adult) or Child and Adolescent intake packet
(Please fill out before your session for review with Lynn after which she will keep as part of your file)
2. Lynn's Disclosure Statement and Teletherapy Informed Consent Form
(Please fill out before your session for review with Lynn after which she will also sign and keep as part of your file)
